Initial Registration to Perform Attest/Comp Services as an Individual Practitioner

This form is designed to allow individual practitioners to make application to provide attest and/or compilation services in West Virginia. If your accounting practice is organized as a sole-proprietorship and you have not organized as an AC or a PLLC, you are eligible to complete this application.

The Application Processing Fee is $85.00

During the application process, you will be required to upload documentation that you are currently enrolled in a Peer Review Program. Please scan the documentation into an Adobe .pdf file in preparation for the upload process.
