Original Certification

The Board shall issue an original certificate to an applicant who demonstrates that he or she:

has met the education, examination and experience requirements;
has submitted an application on a form prescribed by the Board;
is trustworthy and of good moral character;
has paid the appropriate fee prescribed by the Board; and
has submitted a state and national criminal history record check
