Examination Forms

Listed below are the Exam Application Forms needed to:

  • make an initial application to sit for the CPA Examination
  • apply for a transfer of credit toward certification, or
  • make the Board aware of your Intent to Sit for a section or sections of the CPA Examination

In order to use these forms, you must have Adobe Acrobat Reader installed on your computer. You can download the Reader at www.adobe.com.  

Initial Application  For exam candidates making an initial application for the examination or for exam candidates who have missed six consecutive examination windows and must now submit a new application to establish eligibility. THIS FORM HAS CHANGED.  PLEASE REVIEW THE ATTACHED INFORMATION CAREFULLY.

Intent to Sit   For exam candidates who have not missed six consecutive examination windows and need to sign up to sit for a part or parts of the examinaiton.   THIS FORM HAS CHANGED. PLEASE REVIEW THE ATTACHED INFORMATION CAREFULLY. 

The Board cannot accept applications via e-mail or fax. The completed form must be printed and mailed to the Board office along with your fee.
