 CPE Forms and Extension Request Form



Important CPE Reporting Information

As most of you know, Board staff believed our new e-licensing system would be live by now. However, there are glitches with the CPE Reporting application that prevent us from releasing it for "go live" at this moment.

While Board staff continues working with the developer to provide our licensees with an online CPE form,  Board staff recommends that, as in the past, licensees 
choose either the Adobe or Excel file below for reporting purposes this year.

You may either e-mail the completed form to:

or mail to:  West Virginia Board of Accountancy, 405 Capitol Street, Suite 908, Charleston, WV 25301

Certificates of Completion

 
FYI: The Board no longer requires one to submit the Certificate of Completion

for each self-study course completed with your reporting form each year; however, you must keep all CPE documentation for a minimum of 4 complete calendar years in case of CPE audit.
